If you have a medical condition that can be alleviated through the use of medical marijuana, obtaining your medical marijuana card is a straightforward process. Outlined below is an overview of the application process.

Colorado Medical Marijuana Card Registration Requirements:
- Complete an application form for a medical marijuana registry or identification card.
- Sign, date and have the registry application form notarized. We can do that for you!
- Have the physician sign and date the physician recommendation form.
- Photocopy a legible copy of an approved photo ID that establishes Colorado residency.
- Send in a non-refundable application fee of $35 (check or money order payable to CDPHE)
- Send all items above to:
Colorado Department of Public Health and Environment (CDPHE) in care of
Colorado Medical Marijuana Registry

Five Steps to getting your MMJ Card
- Schedule an appointment with Dr. Nelson, 970-799-2697.
- Meet with doctor to review your medical condition and receive your official Doctors Certification.
- Fill out a Medical Registry and have your signature notarized. (Bring your paperwork to the Acceptus Group and we will notarize and send out!)
- Make copies of all information, we can do this too!
- Submit your application to the state with a copy of your valid ID, we are happy to do this too!
